Think of this phase as embarking on a familiar path that has unexpectedly transformed. Prostate cancer treatments, while life-saving, can impact sexual function in ways that may feel challenging. Understanding these changes is the first step toward healing. 

Immediate Effects

Most men experience some degree of erectile dysfunction (ED) after prostate cancer treatment, particularly following surgery (radical prostatectomy) or radiation therapy. The extent and duration of ED vary depending on factors like the type of treatment, age, and overall health. 

  • Surgery (Radical Prostatectomy): ED often occurs immediately after surgery due to temporary nerve and tissue trauma, with nearly all men experiencing some ED initially (Movember, 2023). 
  • Radiation Therapy: ED may develop gradually over time, sometimes progressing over a year (American Cancer Society, 2023). 
  • Hormone Therapy: Loss of libido and ED can emerge quickly with hormone therapy and may persist (Cancer Council, 2023). 

Recovery Timeline

Healing is deeply personal, and recovery of sexual function takes time: 

  • Within One Year: Many men experience substantial improvement, especially those with intact nerves. About 40-50% regain their pre-treatment function within a year (National Cancer Institute, 2023). 
  • Up to Two Years: Recovery can continue over two years, with approximately 30-60% regaining their previous level of function by the second year (Movember, 2023). 
  • Age Factor: Younger men often have better outcomes. For instance, men under 65 have a 59.3% potency rate after one year, compared to 31.3% for men over 75 (Fode et al., 2022). 

Movember’s Role in Sexual Health Recovery

Movember is a global charity dedicated to men’s health, focusing on post-cancer sexual health recovery. 

  • Sex and Intimacy Guide: This guide provides practical strategies to help recover sexual well-being and encourages shared decision-making among clinicians, patients, and partners (Movember, 2023). 
  • Sexual Health Guidelines: Movember worked with international experts to create clinical guidelines, including 47 recommendations for healthcare providers to improve care for prostate cancer patients (Fode et al., 2022). 
  • True North Program: This initiative offers resources to help men take charge of their recovery, available on the True North website (Movember, 2023).

Enhancing Recovery – Strategies for Sexual Health

Several approaches can support sexual function recovery: 

Medical Treatments

  • Oral Medications: Drugs like Viagra, Cialis, and Levitra can enhance blood flow and are effective for about 75% of men post-nerve-sparing prostatectomy or targeted radiation (American Cancer Society, 2023). 
  • Penile Rehabilitation: This may include regular oral medications, injections, or vacuum devices soon after treatment to promote blood flow and prevent long-term ED (Cancer Council, 2023). 
  • Injectable Treatments and Mechanical Devices: For those unresponsive to oral medications, options like direct penile injections, vacuum devices, or penile implants may help (National Cancer Institute, 2023). 

Lifestyle Modifications

  • Healthy Weight & Exercise: Regular exercise and maintaining a healthy weight can positively impact erectile function (Movember, 2023). 
  • Quit Smoking & Limit Alcohol: Smoking damages blood vessels, while excessive alcohol can interfere with sexual performance (American Cancer Society, 2023). 
  • Manage Stress: Practices like meditation, yoga, or counseling can alleviate stress that may impact libido and erectile function (Movember, 2023). 

Pelvic Floor Exercises

Kegel exercises strengthen the muscles supporting erections, often leading to improved sexual performance (Cancer Council, 2023). 

Intimacy Beyond Intercourse

Rediscovering intimacy can mean exploring new forms of connection: 

  • Emotional Closeness: Open communication builds understanding and support, turning challenges into growth opportunities. 
  • Non-Penetrative Activities: Engaging in massage, sensual touch, and other forms of affection can enhance intimacy. 
  • New Experiences: Being open to different forms of expression can bring you and your partner closer. 

Psychosexual Counseling – The Power of Support

Psychosexual counseling can be crucial for those recovering after prostate cancer: 

  • Managing Psychological Impacts: Counseling can help address body image, self-esteem, and the emotional aspects of sexual recovery. 
  • Education & Expectation Setting: Counseling provides realistic recovery timelines and helps manage expectations. 
  • Communication Support: It enables open communication between partners and explores alternatives to traditional intimacy.
